MIKE DEL ANGEL BRAZILIAN JIU JITSU WAIVER

I realize that serious injury is commonplace in martial arts academies such as the one I am herby entering; I might be disfigured, disabled and/or rendered unable to work again. I realize that regardless of how this consequences might occur whether it be the results of an opponent’s actions, the actions or inactions of an instructor or assistant, the condition of the mats used, the conduct of a non-participant or some other reason, by entering into this agreement I accept the risk of entering and being present at the classes and give up and waive all claims I or anyone acting on my behalf or through me might have against Mike Del Angel Brazilian Jiu Jitsu LLC, Hermilo Del Angel, Petrayris Del Angel, the instructors or any other employee and representatives for any injury regardless of its nature, effect on me as a result of my participation in class.

I Authorize and agree to hold harmless, whatever medical personnel may be present at the academy to take any action necessary, should I become injured.

I represent that I am in good health; that I am not presently, nor will I participate while under the influence of any drug or medication; that no one affiliated with this academy has encouraged me to enter or made any representations regarding my fitness or ability to join in (participate).

That I have read and understood every provision of this release; and that I am legally competent to and freely enter this waiver, release, and assumption of risk agreement. I also understand that I’m releasing Mike Del Angel Brazilian Jiu Jitsu LLC from liability resulting from illness such as communicable diseases such as COVID-19.

I also hereby grant Mike Del Angel Brazilian Jiu Jitsu LLC permission to use my likeness in photograph(s) in any publications or publicity materials in perpetuity. I will make no monetary or other claim against Mike Del Angel Brazilian Jiu Jitsu LLC for the use of the photograph(s).
